Healthier Communities Map

Parks and places to play, health care facilities that offer free or reduced cost care, farmer’s markets and fruit and vegetable stands and retailers that accept SNAP are important assets for healthy living.

This map is home to resources, which we sometimes call community assets, available to Edgecombe and Nash counties. They include the assets that can help support a healthy and active life. Click THIS LINK to open the map in a new browser window.

The information presented in this map was gathered by the North Carolina Institute for Public Health. Assets were mapped using the best available data as of May 2018. To see the sources, click on the “i” in the white circle located in the legend in the upper right hand corner of the screen.

NCCARE360

NCCARE360 is the first statewide coordinated care network to electronically connect those with identified needs to community resources and allow for a feedback loop on the outcome of that connection.

United Way 211

The United Way’s 211 website connects people to services and assistance with housing and shelter, utilities, healthcare, food, mental health and substance use, clothing and household goods, aging and disability services, employment and income, and tax assistance.

Additional resources can be found at the Nash and Edgecombe County Health Departments, including services for child health, family planning, immunizations, and care for elderly or disabled residents.
